As required by Kansas Statutes 72-5209, 72-5214, and 72-503, 106, the following is a list of items required to be in all student health files.
● Proof of required immunizations
KANSAS K-12 SCHOOL IMMUNIZATION REQUIREMENTS FOR 2022-2023 SCHOOL YEAR
EARLY CHILDHOOD PROGRAMS OPERATED BY SCHOOLS IMMUNIZATION REQUIREMENTS FOR 2022-2023 SCHOOL YEAR
● Health assessment completed by a licensed health care provider within the past 12 months of the first day of school USD 402 health assessment form - (will accept any form, provided it is signed by a health care provider)
● Copy of student’s official, state-issued birth certificate (cannot be a hospital copy) As stated in Kansas Statute 72-6262, a child may be exempt from immunization if:
● An annual written statement signed by a licensed physician stating the physical condition of the child to be such that the tests or inoculations would seriously endanger the life or health of the child
● A written statement signed by one parent or guardian that the child is an adherent of a religious denomination whose religious teachings are opposed to such tests or inoculations.
Please provide the documentation before the first day of school. Your child will not be able to attend school after September 1, 2022 if all required documentation is not turned in. You may fax or email documents to the school your child will attend in the fall.
